Ape Escape, the original one on PSX. All-time favorite.
EDIT: Turok: The Dinosaur Hunter on N64 was a great game too. Oh, the old days.

It's tough to pick an all-time favorite because so many games were great for their time. To list a few though...

Mega Man X - X3 on SNES --- Mega Man X is still my hero!
Phantasy Star Online (PSO) on Sega Dreamcast --- Loved the atmosphere of the game, and it got me into Online gaming.
Mario Kart/Perfect Dark/007 Goldeneye/Smash Bros/Ogre Battle all on Nintendo 64 --- Great games of many genres.
Final Fantasy VII, VIII, IX, and X --- VII and X are probably my two favorites, but they were all good!
Halo 1,2,3 on Xbox/360 --- Great series of games that i think redefined online shooters.
Ninja Gaiden/Ninja Gaiden II on xbox/360 --- Very hard games but so good too!! Ninjaz4lyfe!!
Left4Dead on 360 --- Best zombie apocalypse game made to date in my opinion.

I think that covers most of my favorites

, Kingdom Hearts series, Dead Space, GTA series, Harvest moon wonderful life, FF7, L4D, Pokemon for my DS (that game will seriously keep you busy on damn long plane rides and waits in airports!) God of War, and a bunch others I can't think of right now
